TCTM President's Grant Application Form

Eligibility

Grants of $600 for the recipient to attend CAMT will be awarded to current mathematics educators with a district or campus affiliation with more than five years teaching experience in Texas. Previous winners of this grant are not eligible to receive it again. Winners will be determined by random drawing of names and will be notified by telephone or email no later than April 1, 2012. Winners will be asked to volunteer for two hours at CAMT and will be TCTM’s guest at our reception where the checks will be presented. Good Luck!

Winners must complete a W-9 form, including providing TCTM with a taxpayer ID (Social Security) number prior to receipt of the award at CAMT. Winners are responsible for reporting the amount of this award on their federal tax return, as the information will be reported to the Internal Revenue Service (IRS). Pursuant to IRS regulations, TCTM will provide you with a 1099-MISC for 2012.
